Preseason Info

For the 2025-2026 Season, there are 3 MANDATORY REGISTRATION ITEMS that need to be addressed before your kid(s) can participate on our team.

Pay Fees (Mandatory, with exceptions)
- These two fees can be paid in cash, with 2 checks, or combined into 1 check. Coach Trent will collect the money (do not mail money, physically hand it to Coach Trent) for these fees and will turn them into the school. Be sure to sign the Multiple Receipt Form so our team and the school has record of who has paid.
- The first mandatory fee is $25 for FCPS Athlete Insurance. If your student-athlete has played a sport for any FCPS school in the 2025-2026 school year (typically a fall sport like golf, cross country, soccer, etc.), this fee should have been paid to that team. For this fee, Marching Band is not considered a sport. Student-Athletes who qualify for Free & Reduced Lunch are exempt from this fee.
- The second mandatory fee is the $25 LHS Athletics Fee. If your student-athlete has played a sport for Lafayette HS in the 2025-2026 school year, (typically a fall sport like golf, cross country, soccer, etc.), this fee should have been paid to that team. For this fee, Marching Band is not considered a sport. Student-Athletes who qualify for Free & Reduced Lunch are exempt from this fee.
SwimTopia Registration (Mandatory)
- This will be used to collect info (parent's emails and phone numbers, athlete birthdates, merchandise info, etc) that we will need throughout the season. Click the link above to complete this part of the registration.
- Families with multiple student-athletes signing up for our team only need to submit this form once (this registration is designed on the "family" level and their is an option to add multiple athletes).
Completed Forms Upload Survey (Mandatory)
- This will be used to submit all of the necessary forms that the KHSAA/FCPS/Lafayette requires. There are fillable PDFs of each form linked below. Once all forms are completed, you can submit them using the Completed Forms Upload Survey linked below.
- Families with multiple student-athletes signing up for our team NEED TO SUBMIT THIS FORM ONCE PER CHILD (2 kids means you need to submit this form twice, etc.). In addition to submitting these forms separately, SIBLINGS CANNOT SHARE FORMS, even if their information is the same (each kid must have their own KHSAA Sports Physical, Address Verification Form, etc.).
- Below is a list of the required forms you must submit on behalf of your student-athlete. Here are some notes on each form, but please read each form for its specifics.
  - Middle School Play Up Approval Form: For our 7th & 8th graders only. This must be signed by your middle school principal or athletic director.
  - KHSAA Sports Physical: There have been several iterations and versions of this over the past few years. The one linked below is the correct one. This is a 4 page document (3 pages where you fill out information, the last page is a disclosure). The first 3 pages must be submitted, including the third page which requires a signature from a medical professional. If your child's physical was performed on or after February 1st, 2025, your child's physical will remain valid for our entire season. It is okay to submit a physical that was performed earlier than that, but know that you will have to get an updated physical during our season to remain eligible.
  - Address Verification Form: Confirms the student-athlete lives in the Lafayette School District or that the student-athlete has permission to attend Lafayette HS (via our magnet programs).
  - Media Release Form: Allows Lafayette/FCPS to photograph, video, or interview your student-athlete to be used in the news media or a Lafayette/FCPS publication
  - Student Trip Form: Lafayette/FCPS does not provide transportation to any event. Students cannot transport themselves to any event outside of Fayette County. Parents/Guardians are responsible for their child's transportation to and from events.
  - Swim Test Form: Transy (our "home" pool) administers a Swim Test at the beginning of each season. This involves swimming one length of the pool, followed by 60 seconds of treading water. Student-Athletes must pass this to be allowed on our team. Parents/Guardians must consent to Transy administering this Swim Test. You must select Option 2 (test in shallow and deep water).
